Case Reviews are the method used to re-establish and/or confirm eligibility for financial assistance for open cases. Together with NB Case, case reviews complete the internal control environment for payments to or on behalf of clients. The completion of the Case Review form is considered a re-application for assistance.

The Case Review form is used to verify all information given by clients and to note all changes. The employee completing the review must do so thoroughly, initialing all verifications, and indicating questions which are not applicable to the clients.  For example (N/A).

The declaration on the Case Review form must be read to those signing the document. The review must be signed by all adult members of the household, and attempts must be made to have it signed by any members aged 16 or older who are not attending school full-time.  

When possible, case reviews should be completed in the client's home. If this is not possible, the review may be completed at the SD office.

 

Schedule for Case Reviews

Case reviews must be completed according to the following schedule:

ACTIVE CASES: Every 12 months

TARGET CASES: Every 12 months

All cases must be reviewed within one month of being transferred to Active or Target caseloads.

MAINTENANCE CASES:

  • Blind, Deaf and Disabled: Every 36 months
  • Head of Household over age 50 and/or in receipt of CPP disability benefits: Every 24 months
  • All other maintenance cases: Every 12 months

 

Who Performs Case Reviews

Case managers will complete Case Reviews with their own clients at designated times. This process serves  as an opportunity to review the continuing financial eligibility of clients and assess the need for programs and/or services such as Career Development Opportunities (CDO).

Case reviews by peers are completed on a random sample of cases. This will independently review financial eligibility, adherence to policy, procedures and service delivery standards.
