In order for SD to provide assistance, an applicant must provide or complete documentation where applicable, at the time of the Registration interview.
4.7 - Intake Documentation
- SIN and Medicare cards for each adult (18 and over) member of the household. If not available, two other pieces of identification (Driver's License or Birth Certificate) may be accepted temporarily.
- A Medicare card and Birth Certificate (or Baptism Certificate) for each child under 16 in the household. If the child has a SIN card, this must also be included.
- Verification of Address - this may vary, depending on the applicant's accommodation type:
Rent: (Must be one of the following, or assistance will not be issued)
- a copy of the lease,
- the most recent rent receipt (must be detailed, indicating the landlord's name, address, and phone number, and if heat and light are included), or
- completed Residence Confirmation form;
Boarding: Completed Residence Confirmation form. The applicant must submit this form; otherwise assistance will not be issued.
Own Home with Mortgage: Documentation must be one of the following, or assistance will not be issued.
- copy of mortgage agreement or
- receipt from mortgage holder;
- (A mortgage receipt will be considered inappropriate if it is informally written i.e. on a scrap of paper, or if information required is either unclear or missing.)
Own Home No Mortgage: Documentation must be one of the following, or assistance will not be issued.
- copy of most recent tax bill, or
- copy of deed for property;
Maintenance Only:
- written verification from the property owner, indicating the owner's name, address, and phone number. This must include the terms or conditions of the client residing in the home i.e. the client's responsibilities. Statement of Non-Support, for single parent applicants and for applicants aged 16-18 years. Separation, divorce, or custody papers if available.
- Record of Earnings (Separation Slip) along with last pay sub from last employment.
- Income Verification. If applicable this could include income stubs, a letter from an employer, financial statements, etc. depending on the income type. See the Help Index for details related to specific types of Income.
- Asset verification. This will vary, depending on the asset type. See the Help Index for details related to specific types of Assets.
Documentation is as for all applicants plus there must be documentation confirming:
- if awaiting EI, confirmation of dates of awaiting period
- if awaiting income other than EI, the source of expected income with details re: source, contact person, telephone number etc.
If application is for child care/day care due to medical reasons, the applicant must provide a recommendation from a physician indicating diagnosis and the hours per day and length of time child care is required.
If application is for child care/day care due to training or employment, the applicant must provide proof of the hours of training or employment, length of training etc.
If applicable, documentation must be provided re: the Day Care centre. Information must confirm the child is enrolled in an approved day care centre, and must include dates of enrollment, and cost per day.
For applicants requesting assistance for the funeral expenses of a deceased individual, documentation is required not only relating to the financial situation of the deceased, but also of the family of the deceased and of the applicant.
If the application is for assistance due to a medical condition which prevents them from working for six or more months, the applicant must have a Medical Report completed by a physician. It will be recorded only after such a report is returned.
In addition to the information that is provided by all applicants, those applying for:
- Section 4(4) - Health Coverage or one issue medically related benefits,
- Assistance plus Continuing Benefits of Special Needs for medical reasons
must also provide the following:
- a recommendation from their physician indicating diagnosis, benefits required and
- estimates for all medical benefits
- proof of all expenses and income to be considered for Section 4(4)
Those applying for assistance under Section 4(4) must provide all applicable documentation such as is required from all applicants. They may also need to provide the following, depending on the nature of their request:
- documentation regarding all income, including some income types not usually considered
- documentation regarding the Special Benefit they are requesting
- proof of all expenses to be considered
For applicants to the Prenatal Benefit Program, the existing procedure for obtaining identification will be used except when obtaining a proof of residence, where the following should be considered:
The Residence Confirmation Form is not a requirement for eligibility to this program. A New Brunswick Medicare card with a vaiid date is one indicator that an applicant resides in New Brunswick. A rent receipt, copy of a lease/rental agreement, a copy of the applicant's telephone or hydro bill or written verification of a room and board situation is sufficient verification of residence.
