As of April 1, 2018, adult adoptees and birth parents will be able to obtain forms to apply for access to birth records related to an adoption. These forms will be available on the Department of Social Development’s web page for the Opening of Sealed Adoption Records.

If you are an adult adoptee or the birth parent of an adoptee and would like to apply for information, you must complete the appropriate application form and attach clear copies of two of the following documents:

  • Birth certificate
  • Driver’s license, or
  • Passport.

If you are the adult child of a deceased adult adoptee or the adult child of a deceased individual whose child was placed for adoption, you require identification as well as proof of death. Proof of death may include a copy of:

  • Death certificate
  • Death notice or funeral card; or
  • Obituary.
