Q. What is a Health Coach?

A.  A Health Coach has the training and knowledge to help you develop a personal plan for wellness, while providing motivation and practical support. The Health Coach lives in the community and is knowledgeable about available resources.

Q. What qualifications does the Health Coach have?

A.  The Health Coach’s qualifications include post-secondary education in fields such as Health Promotion, Social Science, Physical Education or Kinesiology, with related experience in community development, health and wellness promotion, as well as training on the Live Well | Bien Vivre project’s self-management tools.

Q. Where did the idea of a Health Coach originate?

A.  The role of the Health Coach role is modeled on the United Kingdom’s successful Health Trainer Program.  Through communication and collaboration with U.K. program leaders, Live Well | Bien Vivre has drawn on the expertise and lessons learned to pilot our own Health Coach model.

Q. How do I access the services of a Health Coach?

A. Individuals may either self refer or are referred through the risk assessment clinics offered by the Canadian Diabetes Association. 

Q. Will my personal information be shared with anyone other than the Health Coach?

A.  The confidentiality of all data collected for this project will be maintained according to the privacy policy of the Canadian Diabetes Association. Client data will be reported in a format where no participant can be personally identified. None of your personal information will be reported unless you provide written consent, allowing us to share your information. 

Q. How do I contact a Health Coach?

A.  The Health Coach will work with you by telephone or face-to-face.  They will meet you in a community location that’s convenient for you.
