Contaminated Sites Management Process

The following documents outline procedures and requirements for managing contaminated sites in accordance with provincial requirements.


Contaminated sites program ministerial protocols

The six Ministerial Protocols provide information and requirements for the entire Contaminated Sites Program. Protocols 400, 500, and 600 specifically deal with the assessment, remediation, risk assessment, monitoring and file closure for Contaminated Site Management (CSM) files.


Forms

Notification of historical contamination

Submit form CSM-FRM-100 to notify the DELG of the discovery of historical contamination

Report submission

Submit form CSM-FRM-200 with ESA, RAP, Monitoring, or Closure Reports

Monitoring well exclusion

Submit form CSM-FRM-300 to request an exemption to drilling monitoring wells at a site

Access denial

Submit form CSM-FRM-400 when denied access to a source or third party property

Request for time extension

Submit form CSM-FRM-500 to request an extension of the regulated ESA or Closure Report deadlines

Monitoring well decommissioning

Submit form CSM-FRM-700 to notify the DELG of well decommissioning completion

Record of site condition (long)

Submit CSM-FRM-600 (long) for sites with more than 3 PIDs
