The New Brunswick Cancer Network (NBCN) in collaboration with both Horizon and Vitalité Health Networks has established the provincial, community-oriented Cancer Patient Navigation Program. The oncology nurse navigators, along with the existing pediatric oncology navigators will work with patients and their families, primary care providers, the interdisciplinary oncology team, and community support agencies. The Program will provide assistance and support throughout the cancer continuum, especially in the pre-diagnostic and post-treatment transition to cancer survivorship.
Referral forms are available to health care providers at both Horizon and Vitalité Health Networks.
